My observations are that the analysis of failed projects and most of the advice on how to avoid a failed project are sadly inadequate.
Most of the blame can be attributed to the requirements gathering approach. My view is that the real cause is the use of requirements to define the problem and solution.
This site explains my position and puts forward an alternative approach.
